We realize that, at first glance, it may seem a little long, but it will be very helpful for us when it comes to helping choose the right puppy for you and your family.

Just a note before you get started - this questionnaire is not a test to qualify you as an approved home for one of our puppies. We get a lot of inquiries and this is our cheat sheet for getting to know you.

We raise all of our puppies in our home and we hope you love your puppy as much as we do.

We do everything we can to make great matches for each and every puppy we raise.

We want to ensure both puppy and family are 100% happy with the placement!

Every family is different, and there are no right or wrong answers on this form.
